Since the Institute of Medicine Report in 1999, To Err is Human, that reported that upwards of 98,000 people die each year due to medical error, healthcare providers have struggled with ways to enhance patient safety by focusing on structural design and by attempting to create a culture of safety in the healthcare workplace.
