Kenneth D. Gossett, Ph. D., CEO of Guardian created the Quality of Life Assessment Tool that has been used to measure consumer outcomes that result from services and supports provided by a Home Health Care Agency.

The Quality of Life instrument contains forty (40) questions that address four domains: Physical Well-Being, Psychological Well-Being, Psychosocial/Emotional Well-Being and Personal Well - Being.
