CDPHP Honored For Best Disease Management Program In Upstate New York
Health and Wellness Efforts Recognized For Fifth Consecutive Year

For the fifth consecutive year, CDPHP® has been recognized for offering the best disease management (DM) program in upstate New York by Health Industries Research Companies (HIRC), a national, independent research organization serving the health care industry. CDPHP was the only health plan in the Capital Region to receive this award, with no other health plans north of New York City or east of Buffalo recognized. Less than six percent of organizations are honored, and only about a quarter of those organizations have been honored every year, as CDPHP has.

Certification ensures accurate disease management and wellness reporting to employers from health plans and vendors. Savings Measurement Validity
Certification from the Disease Management Purchasing Consortium (DMPC) for the valid savings measurement, an industry standard metric used to calculate savings measurements from disease management programs.
Best Disease Management Program
Honored for five consecutive years as having the best disease management program in upstate New York by HIRC.
