New UCSF Global Health Initiative Will Help Bridge Gap Between Knowledge, Action

UCSF and SEEK Development, a global health and development consulting group based in Berlin, Germany, have launched an international partnership that aims to improve global health by helping to turn scientific evidence into policy and action. The Evidence-to-Policy Initiative, or E2Pi, officially launches this week in San Francisco and Berlin, Germany.

UCSF and SEEK Development, a global health and development consulting group based in Berlin, Germany, have launched an international partnership that aims to improve global health by helping to turn scientific evidence into policy and action. The Evidence-to-Policy Initiative, or E2Pi, officially launches this week in San Francisco and Berlin, Germany.

“For many of the world’s major health challenges – including HIV, tuberculosis, malaria, and maternal and child deaths – a failure to translate evidence on what works into practical health policies is costing lives,” said Sir Richard Feachem, director of the UCSF Global Health Group, which is leading the collaboration.

E2Pi aims to help address some of these challenges by closing the gap between what is known and what gets done in practice.

“The gap between available scientific knowledge and its application in solving global health problems is one of the reasons why poor countries suffer such a high burden of disease and premature death,” said Feachem, who was formerly the founding executive director of the Global Fund to Fight AIDS, Tuberculosis and Malaria. “E2Pi will help to address this issue by conducting rigorous and independent analyses of the available research evidence leading to specific, action-oriented recommendations to policy - and decision-makers.”

Funding for the initiative’s first three years is provided by a $3.2 million grant from the Bill & Melinda Gates Foundation.

E2Pi has already produced a series of briefings summarizing evidence of progress in tackling eight global health priorities (HIV/AIDS, tuberculosis, malaria, polio, immunization, maternal and child health, nutrition, and neglected tropical diseases). These briefings supported the launch of the Living Proof Project, an advocacy campaign spearheaded by the Bill & Melinda Gates Foundation that aims to sustain and increase investments in global health (www. livingproofproject. org).

E2Pi is led by a team of policy and research experts, supported by a wider international network of global health specialists. The U. S. hub for the project, based at the UCSF Global Health Group, is led by Gavin Yamey, M. D., a physician and former senior editor at two influential open access global health journals: PLoS Medicine and PLoS Neglected Tropical Diseases. The European hub, at SEEK Development, is directed by Christina Schrade, who has held senior positions at the Global Fund to Fight AIDS, TB and Malaria and was a manager at McKinsey & Company, a leading management consulting firm.

Past-year use of the club drug among people over 12 declined by more than one million

The National Institute of Drug Abuse (NIDA) issued its latest Research Report this year focusing on ecstasy abuse, citing that past-year use decreased from 3.2 million people over the age of 12 to 2.1 million from 2002 to 2003. The same report said past-month users dropped by more than 200,000.

This is good news, but ecstasy (MDMA) abuse has spread to a wide range of other settings and demographic subgroups than just raves in the past few years. Despite growing evidence of its harmful effects, ecstasy still has a deceptive reputation as a “safe” drug among its abusers. The idea of just taking a pill with a design stamped on it makes it more attractive and appear less harmful. Additionally, popping a pill is more socially acceptable than snorting, smoking or injecting a drug.

While overall ecstasy use among teenagers has declined over the last couple of years, a survey of adolescents and young adults who reported ecstasy abuse found that more than a third of them were addicted and in need of treatment.

Due to the stimulant properties and the increased physical activity often associated with taking the drug, the result can be a dangerous increase in body temperature that produces dehydration and increased heart rate, while potentially leading to stress on the heart and cardiovascular failure. Additional potential adverse effects include nausea, chills, sweating, involuntary teeth clenching, muscle cramping and blurred vision.

There are other short term effects of the drug such as impaired memory, difficulty processing information and trouble performing skilled activities such as motor vehicle operation. According to NIDA, ecstasy users report feeling anxiety, restlessness, irritability, sadness and depression in the week following moderate exposure to the drug.

Regular MDMA abusers demonstrate elevated levels of anxiety, impulsiveness, aggression, sleep disturbances, lack of appetite, and reduced interest in and pleasure from sex. The cycle of addiction continues by taking more of the same drug to escape the condition brought on by that drug, all the while causing more damage mentally and physically.

One former ecstasy user summed up the drug's effects oh him saying, "I felt so much emotional pain and was so depressed that I wanted to end my life and take all of the world's pain with me." The young man has since become drug-free, but not without help.

In the latest issue of Nursing Education Perspectives, the respected peer-reviewed bi-monthly journal of the National League for Nursing, Dr. Jane Sumner, a member of the NLN Board of Governors and associate professor and acting associate dean for undergraduate programs at the Louisiana State University Health Sciences Center School of Nursing, recounts the dramatic story of how students, faculty, and administration struggled to function off-site for a full year before returning to their home in New Orleans, following the devastating destruction wrought by Hurricane Katrina.

From the time the levees were first breached in August 2005, recalled Dr. Sumner, the commitment to educating the next generation of nurses and nurse educators remained unwavering, even if that meant operating for a time in makeshift facilities and relying on the kindness of colleagues in other academic and clinical settings for a variety of critical needs.

Of the year spent in Baton Rouge, Dr. Sumner writes:

"It was an odd experience to teach in a movie theater. We felt dwarfed by our PowerPoints, which loomed overhead on a huge screen, while with no place to put material we did an odd sort of juggling dance to follow our own notes, manage the PowerPoint, and ensure that our students remained engaged.

"But, even as we functioned during that first year, we discovered the loneliness of not having a formal organization and a building to go to on a regular basis. We had not appreciated the function of the ordinary socialization that happens when colleagues gather."

While now back in New Orleans, a city far from restored to normalcy, the LSUHSC School of Nursing continues to provide a first-rate education. As Dr. Sumner observed, program undergraduates are returning to take master's and doctoral level courses, in tribute to their remarkable teachers who have modeled professional dedication. She noted:

Once again we are using evening and night shifts with clinical part-time faculty. But we are managing, and the students are gaining excellent experience. They are learning, in a very direct way, the flexibility, adaptability, and agility needed for practice in nursing today.

Girlstart, is an Austin, TX-based nonprofit organization created to empower middle and high school girls to excel in math, science, and technology. In the summer of 2008, the AMD Foundation's grant will enable 60 Girlstart participants to attend a program focused on games with social content. As a capstone project, girls will be creating a social awareness event in Teen Second Life, a virtual gathering place for teens 13-17 all over the world to make friends, play, learn and create. The Girlstart team will identify a social issue of importance to them and create an event in Teen Second Life that will help raise awareness and inspire action around the issue. Global Kids, is a Brooklyn, NY-based nonprofit organization that seeks to transform urban youth into successful students and global and community leaders. Through its grant to Global Kids' Playing for Keeps program, AMD has joined The Microsoft Corporation in enabling 20 young people from underserved communities to work with game developers to develop, create and distribute a game about the heroic role of residents following Hurricane Katrina. Last year, young people worked with developers in the Playing for Keeps program to create the game Ayiti: The Cost of Life which allows players to assume the role of impoverished people living in rural Haiti with the goal of meeting some key health, education and quality of life challenges. Institute for Urban Game Design, is a Washington, DC-based nonprofit organization teaching science, technology, engineering and math (STEM) skills through the hands-on creation of digital games. Beginning in the summer of 2008, the AMD Foundation's grant will enable IUGD participants to apply their learning in 3-D modeling, animation and computer programming to the development of a game focused on the issue of energy usage. Students will learn about and explore the social issues associated with different types of energy. Science Buddies is a national, non-profit organization based in California's Silicon Valley offering a variety of web-based tools that help K-12 students explore science through research-based projects often done at Science Fairs and other school and community events. AMD Foundation's grant will enable Science Buddies to launch a Video and Computer Games Interest Area on its site aimed at helping students understand and practice what is required to design digital games. AMD volunteers will work with Science Buddies staff scientists to develop project ideas to spark student interest in exploring topics such as human behavior in games, ergonomics, game design and programming and the incorporation of social or educational content in games. As part of launching the Games for Change Festival, the AMD Foundation is sponsoring "Let the Games Begin," a day-long workshop for nonprofit organizations focusing on how to create social issue games. The workshop will feature interactive lectures by some of the nation's leading authorities on social issue game development and cover fundamentals such as game design, fundraising, evaluation, youth participation, distribution and press strategies.

Following the Festival, the AMD Foundation and Games for Change plan to co-produce a how-to digital toolkit for nonprofits that includes examples of games with social content, interviews with key experts and additional guidance for nonprofits creating social issue games for the first time.

AMD is also working with PETLab, a joint project of Games for Change and Parsons The News School, to create a social issue game development curriculum for youth. The curriculum is expected to be piloted in the fall of 2008.

Keystone Mercy Health Plan, the largest Medical Assistance managed care organization in Pennsylvania, will partner with Wachovia Bank, a Wells Fargo Company, and Greater Philadelphia Health Action, Inc. in a day-long construction of a new, ‘safe’ playground at Woodland Academy Child Development Center in Southwest Philadelphia, as part of Keystone Mercy’s Safe Playground program. The program’s mission is to reduce the number of preventable injuries suffered by children, as well as addressing childhood obesity by encouraging healthy, physical activity through the construction of safe play areas. More than 50 volunteers will participate in the ‘build,’ including Anne Morrissey, executive vice president and chief operating officer, Keystone Mercy Health Plan; Ron Heigler, chief executive officer and executive director, Greater Philadelphia Health Action, Inc.; Vince Liuzzi, regional president, Greater Philadelphia/Delaware, Wachovia Bank, a Wells Fargo Company; and Sharon Neilson, director, Woodland Academy Child Development Center.

The groundbreaking ceremony will begin at 7:30 a. m. on Friday, June 18, 2010, with the construction of the playground to begin immediately thereafter.
Keystone Mercy Health Plan and PA State Rep. Kenyata Johnson will hold a ribbon-cutting ceremony at 2:45 p. m. to celebrate the completion of the 1,300 square foot playground. After the ribbon cutting, children from the academy will get to play on the newly constructed play area.

Woodland Academy Child Development Center provides day care and youth services for children ages six-weeks to 14 years old. The center opened in 1990 and accomodates 36 kids, today.

In a 2005 survey of nationally recognized pediatricians conducted by Harris Interactive, 96 percent of the respondents said they believed that activity on playgrounds benefits the mental and social development of children, 89 percent said that play keeps children from becoming overweight and 82 percent agreed that play develops a child’s problem-solving skills, which may lead to increases in academic performance.

This will be the eighth playground built since Keystone Mercy Health Plan launched its program in 2007 and will be the fourth built in Philadelphia. The company and its partners have also built similar playgrounds in Chester Township and Norristown.

A new health care plan, seminars on advanced business solutions, a hotly-contested election and a benefit raffle for hurricane victims all highlighted the event. PESA is the nonprofit trade association comprised of over 800 high-volume eBay sellers representing over seventy million eBay transactions and $1 billion in eBay sales annually.

PESA’s Atlanta Summit in March was attended by nearly 200 of the largest eBay sellers, and was featured in the heralded CNBC special, “The eBay Effect – Inside a Worldwide Obsession,” aired in June and July. This weekend’s event had a similarly robust showing of large sellers who average over $1 million/year on eBay, just topping Atlanta’s attendance.

Bill Cobb’s keynote address, representing the first time a direct report to eBay CEO Meg Whitman has formally addressed the nonprofit trade association, and the participation of so many top eBay managers, is seen by many PESA members as a high water mark in the relationship between eBay (www. ebay. com) and its professional selling community. Recent studies have revealed that over 700,000 Americans sell professionally on eBay.

PESA Executive Director Jonathan Garriss commented, “At the beginning of 2005, eBay made a promise to listen to its community more intently. With the participation of Bill Cobb at our Fall Summit, it is clear that the fulfillment of that promise is being led from the top.”

That sentiment was echoed by many PESA members. According to Garriss, the agency “will work with our members to identify their specific needs and help obtain competitively priced, high-quality insurance including health, dental, vision, property, casualty, liability, and workers compensation. For health insurance in particular, the next step is the creation of a national healthcare program allowing us to take advantage of our size such that we can implement an even more cost-effective program.”

Other topics explored in Summit plenary sessions included shipping solutions, buyer confidence, accounting and human resources, and selling on Amazon. com. Member roundtable discussions, in which the leading professional eBay sellers share and learn business-advancing insights, were a hugely popular component of the event. PESA Chairman and Founder Joe Cortese opened the proceedings on Friday, and Garriss gave a state-of-the-organization welcome on Saturday.

The tool can help employers save millions by guiding disability case managers toward shorter recovery times.

Employees on medical disability leave typically receive case management services from their employer, a third-party administrator or insurance company. Disability case managers (http://www. reedgroup. com/) work with physicians and other professionals to ensure that employees receive the treatment they need to return as quickly as possible to full productivity.

The caseworkers who manage these disability cases typically rely on standard guidelines for return-to-work durations. Some standard durations are simply statistical averages based on previous cases.

But employees are not averages, and many factors can influence the time away from the job that an individual will need to recover. Emotional stress (http://www. mdguidelines. com/anxiety-disorder-generalized) can play a huge role in recovery times for physical injuries. Co-morbid conditions (e. g., diabetes (http://www. mdguidelines. com/diabetes-mellitus-type-ii)) can play a large role. A patient's age and job classification also matter.

With its new Predictive Modeling Tool, MDGuidelines offers case managers two sets of return-to-work durations. One set of durations, a unique result of Reed Group's clinical consensus process, provides minimum, optimum and maximum durations for physiological recovery times, an idealized figure that "factors out" the non-medical complexities of real-world disability cases. The second set, achieved via the Predictive Modeling Tool, creates a benchmark that case managers can weigh against the physiological durations.

"Case managers now can have a benchmark--based on the patient's actual conditions--that they can use to gauge the influence of their intervention," says Seymour.

For example, MDGuidelines lists the optimum duration for physiological recovery time for a bimalleolar (closed) ankle fracture as 14 days. The Predictive Modeling Tool calculates that a 30-year-old male with a sedentary job class and a history of episodic mood disorders usually takes 48.5 days to return to work.

Using the two sets of durations, a case manager could easily see that making sure the worker has appropriate behavioral support early in the healing process could bring the absence duration closer to 14 days instead of 48.5 days.

"The difference between the physiological recovery time (e. g., the time it actually takes for a broken ankle bone to heal) and the normative predicted duration, aside from medical co-morbidities, can be termed the psychosocial part of the duration," says Seymour. "The better our tools can help case managers predict that psychosocial element, the more they can address issues early on in the case to get employees back to full productivity sooner. If an employer compares that difference in physiological recovery times versus normative recovery times, they can directly calculate how much money they can save with good case management."

Seymour says that while the Predictive Tool is directly available on the MDGuidelines Web site, the real cost-savings traction occurs when MDGuidelines data is directly integrated with employee information in case management or electronic medical records software.

He cites three examples:
A corporate disability manager visits the "Medical" tab in her case management software. The predicted return-to-work duration is contrasted with the appropriate physiological recovery time and a dramatic difference is noted. She can see the factors that are contributing to the long prediction, and is shown, via a quick calculation, that intervention can save thousands of dollars by managing to the optimum duration.

A physician treating an employee patient for a potential disability would automatically have the predictive and normative return-to-work durations available as part of the patient's electronic medical record. So, instead of dashing off the standard 'two-weeks off' prescription, the physician would have actual guidelines to inform the prescription for time off from work.

An actuary for a large insurance company reviews her case managers' case outcomes versus the integrated MDGuidelines benchmarking data, all of which is available at her fingertips within her on-site database. She identifies ten common disability conditions that have a high psychosocial factor (e. g., fibromyalgia). The disability manager asks her case managers to pay special attention to managing those ten disability types, making sure employees get the support they need to return to full productivity sooner.

"Static disability duration (http://www. mdguidelines. com/) guidelines simply can't provide the cost benefits and employee health benefits that these new integrated and predictive duration tools can," says Seymour. "This predictive durations content should used at every point in the disability process to drive better outcomes, both for employers and their employees."

Visioning Tibet chronicles the passion of ophthalmologist Marc Lieberman, founder of the Tibet Vision Project. His mission: to end preventable blindness in Tibet — which has the highest rate of untreated cataract blindness in the world — by 2020. Bringing light where there was once darkness, Lieberman’s work has been recognized by the American Academy of Ophthalmology, which named him 2003 Humanitarian of the Year.

The film tells the stories of two of Tibetans — Karma and Lhasang — who have one last chance at restored sight.

Karma, 52, is from a small northern Tibetan village. A farmer, he works the land that his family has farmed for generations. Karma has only left home to barter in a neighboring village and to make religious pilgrimages. For two years, he has been gradually losing his eyesight, preventing him from working in the fields. His one wish, if his sight is restored, is to make a pilgrimage to a lake sacred in Tibetan Buddhism. Lhasang, 56, is patriarch of a nomadic family. Like his ancestors before him, he herds yak and goats on the plains of northern and central Tibet. Lhasang’s blindness has made him unable to provide for his family, darkening both his mood and outlook on life.

The film follows the two men as they make the arduous journey to a remote clinic in the hopes of having their sight restored by Tibetan doctors, who have received technology and skill training through the Tibetan Vision Project.

In order to shoot the documentary, Solotaroff had to keep crew and equipment to a bare minimum — just a director/producer and camera/sound person — in order not to raise suspicions of the Chinese authorities. Local volunteers were used as grips and camera assistants. If the Chinese officials had known that the crew was shooting a documentary, it’s movements would have been closely monitored and restricted. Filming took nearly three years. Visioning Tibet uses breathtaking cinematography to provide a view of contemporary Tibet and its people seldom seen by international audiences

Dr. Marc Lieberman founded the Tibet Vision Project in 1995. The Tibet Vision Project operates as a non-governmental organization (NGO). It is one of the few NGOs or international health organizations able to function in Chinese – ruled Tibet. For the past 10 years, “Dr. Marc” has made biannual one-month trips to Tibet, leading a team of doctors, nurses and technicians he has trained. Together, they run “eye camps” that provide basic eye care and perform up to 120 cataract operations in four days. To date, the project has restored sight to over 3,000 Tibetans. The Project’s 10th anniversary was lauded in the Congressional Record in July.

In the 1980’s, Lieberman became involved in Buddhist meditation and practice and active in Buddhist communities throughout northern California. He organized a series of meetings in the US and India between The Dalai Lama and Jewish scholars and rabbis, during which time he learned of cataract blindness crisis in Tibet. These meetings were the basis of the book of The Jews in the Lotus.

Producer/Director Isaac Solotaroff is an award-winning producer, director and editor. He was co-producer, co-director and editor of his first film, Jews and Buddhism: Belief Amended, Faith Revealed. Narrated by Sharon Stone, the film was chosen “one of the outstanding documentaries of 1999” by the Academy of Motion Picture Arts and Sciences. The film was shown at over 30 international film festivals and screened on PBS.

Solotaroff was co-producer and editor of Los Romeros: The Royal Family of the Guitar, which was nominated for a “best biography” Emmy in 2001 and broadcast nationally on PBS. Holiday season deaths due to Heart Problems increases, can be deterred by early alert warning system to emergency medical crews.

With the busiest time of the Holiday season closing in rapidly, it should be a time of good cheer and anticipation of a new year.

Unfortunately, for thousands of Americans, the Holidays will be their last as they become fatality statistics, a fact evidenced by studies that show deaths from heart problems are more common around Christmas and New YearÂ’s Day than other days of the year.

It‘s those types of statistics that cast a pall on the Holidays for an Atlanta entrepreneur who is a long-standing advocate of early warning systems for emergency medical response teams.

Richard Blackwell is the inventor of NeighborLink (TM) an early alert warning system that can easily be set for such a direct notification of emergency medical response services—faster even then the traditional 9-11 calls—when a heart attack occurs.

Blackwell notes that the most recent studies of deaths due to heart attacks, in which researchers and medical doctors headed by Dr. David Phillips of the University of Columbia at San Diego concluded that there is a definite increase in deaths due to heart attacks during the Holidays, is especially distressing.

"Quicker notification of emergency personnel could help reduce these deaths," he reflects.

When Phillips and his team examined heart-related deaths that occurred among patients who died as outpatients or who died in the emergency department, they found that 4.65 percent and 4.99 percent more heart - related and non-heart-related deaths, respectively, occurred than would have been expected.

Further, most of these patients died on December 25, 26 or January 1.

Deaths from causes unrelated to heart conditions were also more common on these three days, which Phillips described as "the most popular days of the year to die."

“It is ironic that while not speculating on the specific cause of the heart attacks, the researchers did point to postponing medical care or, equally important the lack of access to medical care as contributing to the victim's death," Blackwell says.

The group suggested that staffing changes that typically occur during the holiday season, including scheduling changes for doctors and other health care professionals creates delays in addressing concerns.

“Add to this the life-saving implications of the need for immediate notification of all available resources, and it is evident that many deaths can be avoided with such emergency communication capabilities.”

He also pointed out that the doctors doing the research urged Holiday travelers to know about emergency medical resources available when traveling.

”Mobility is an important feature of the NeighborLink (TM) early alert warning system,” Blackwell explains. “It fits on a keychain and is easily adaptable for use in a destination city.”
