Two Years After Injury to Her Husband, Lee Woodruff Shares the Lessons She Learned After Becoming an Instant Caregiver

One January day in 2006, Lee Woodruff was enjoying Disney World with her children. The next, an early-morning phone call from ABC News President David Westin thrust her into the role of "instant caregiver." Her husband, Bob, then co-anchor of World News Tonight, had suffered traumatic brain injury from an improvised explosive device while covering the war in Iraq.

In an exclusive interview with Editor-in-Chief Susan Strecker Richard in the March/April issue of Caring Today (http://www. caringtoday. com), Lee Woodruff tells how she processed the information and then began to steel herself for the frightening journey that lay ahead. Woodruff discusses her coping mechanisms, the lessons she learned as a caregiver and how the family is doing now.

Lee Woodruff, who co-authored with her husband the story of their experience, In an Instant: A Family's Journey of Love and Healing, says instincts overrode emotion during the first days after learning of the tragedy. "I just took control in such a way that I think my brain also took control," she explains. "I programmed for the task at hand, marshaled the forces--which were my four kids--to get them where they needed to go, to get them set up. It was a very methodical, very pragmatic approach that didn't include a lot of emotion."

Woodruff is just one of some 50 million Americans in a family caregiving (http://www. caringtoday. com/basic-caregiver-guide/) role, providing non-paid assistance for a family member or friend who needs support, physical aid and/or guidance with health and medical decisions. "Like most of us thrust into this role, I operated from the gut, mostly learning through trial and error, by doing and reacting in the moment," Woodruff notes. Reserve time for yourself each day. Woodruff relied on an hour-long, daily morning swim to stay fit and to clear her mind for the difficult day ahead. Advocate for your loved one. Because one wrong choice could result in disaster, she learned to communicate with the doctors, to question and seek second opinions and to closely monitor her husband's condition and medication schedules. Cash in your "chits." Woodruff was immediately flooded with kind wishes and offers of help. Telling friends and family she would give them an "IOU" to be called in at a future date made her feel like she wasn't asking for favors all the time. Instead, she called in her chits when she really needed help, whether for a hot meal or a ride for her children.

Both organizations have a track record of innovation and this collaboration provides a framework for improving the quality and affordability of care in a patient-centered way.

This contract includes more than 800 physicians affecting more than 106,000 BCBSMA members. Atrius Health has the highest number of physicians of any organization yet to sign the new contract - the contract also doubles the number of BCBSMA HMO members touched by the AQC. With the addition of Atrius Health, the AQC now includes approximately 2300 physicians providing care for more than 210,000 BCBSMA HMO members in total.

Atrius Health is an alliance of five non-profit multi-specialty groups that includes Dedham Medical Associates, Granite Medical, Harvard Vanguard Medical Associates, South Shore Medical Center, and Southboro Medical Group. As the largest independent physician alliance in Massachusetts, Atrius Health medical groups work together to provide an integrated system of care and coordination of treatment for each patient - a structure that directly aligns with the AQC. The AQC aims to reduce fragmentation of care - resulting in better coordination of care for patients and improved patient outcomes.

The AQC is a significant change from traditional fee-for-service contracts. It combines two forms of payment. The first is a global, or fixed, payment per patient adjusted for age, sex, and health status that increases annually in line with inflation. The second payment includes substantial performance incentives tied to the latest nationally accepted measures of quality, effectiveness, and patient experience of care. The contract's global payment covers all services received by a patient, including primary, specialty, and hospital care. So, when a doctor spends more time with a patient and helps the patient avoid an unnecessary hospitalization, the patient receives better care, the doctor and hospital have the potential to receive performance incentives, and the overall costs are less. Factor spurring formation of inhibitory synapses gives clues to neurologic disorders

Neuroscientists at Children’s Hospital Boston have identified the first known “master switch” in brain cells to orchestrate the formation and maintenance of inhibitory synapses, essential for proper brain function. The factor, called Npas4, regulates more than 200 genes that act in various ways to calm down over-excited cells, restoring a balance that is thought to go askew in some neurologic disorders. The findings appear in the September 24 advance online edition of the journal Nature.

Synapses, the connections between brain cells, can be excitatory or inhibitory in nature. At birth, the rapidly developing brain teems with excitatory synapses, which tend to make nerve cells “fire” and stimulate their neighbors. But if the excitation isn’t eventually balanced, it can lead to epilepsy, and diseases like autism and schizophrenia have been associated with an imbalance of excitation and inhibition. The creation of inhibitory connections is also necessary to launch critical periods -- windows of rapid learning during early childhood and adolescence, when the brain is very “plastic” and able to rewire itself.

Npas4 is a transcription factor, a switch that activates or represses other genes. The researchers, led by Michael Greenberg, PhD, director of the Neurobiology Program at Children’s, demonstrated that the activity of as many as 270 genes changes when Npas4 activity is blocked in a cell, and that Npas4 activation is associated with an increased number of inhibitory synapses on the cell’s surface.

The team further showed that Npas4 is activated by excitatory synaptic activity. "Excitation turns on a program that says, ‘this cell is getting excited, we need to balance that with inhibition,’” explains Greenberg, who now also chairs the Department of Neurobiology at Harvard Medical School.

Finally, the researchers bred live mice that lacked Npas4, and found evidence of neurologic problems – the mice appeared anxious and hyperactive and were prone to seizures.

Greenberg and colleagues are now trying to learn more about the wide variety of genes that Npas4 regulates, each of which could give clues to synapse development and reveal new treatment possibilities for neurologic disorders. “If you have your hand on a transcription factor such as Npas4, new genome-wide technology allows you to essentially identify every target of the transcription factor,” says Greenberg. One such target is neurotrophic factor (BDNF), which Greenberg and colleagues previously showed to regulate the maturation and function of inhibitory synapses.

Children’s researchers Takao Hensch, PhD, and Michela Fagiolini, PhD, also in the Neurobiology program, plan to study the Npas4-lacking mice to see if they have abnormalities in the initiation of critical periods; colleague Chinfei Chen, MD, PhD, will also study the mice, further probing how their synapses develop.

Bricklayers and Allied Craftworkers call for a strike against members of the contractors association.

On July 14, 2005, the Northern California Masonry Contractors Association sent a letter to the Bricklayers and Allied Craftworkers (BAC) Local 3 Union announcing they are cutting off contract negotiations and would lock out all member employees of BAC Local 3 if BAC engaged in a strike against any contractor association member.

Since the contractors were not willing to continue to negotiate, BAC decided to call a strike against members of the contractors association on July 14, 2005, in an effort to get them to return to negotiations.

BAC Local 3 Bricklayers of Northern California have been working without a contract since July 1, 2005. Contract negotiations began May 25, 2005 with the Masonry Contractors Association and the process has faced a few delays. Some of the delays included documents that contained omissions without notation and a misquote of the Industrial Wage Order 16-2001.

Due to an incident on June 9, 2005 the negotiations were further delayed. The incident was severe enough to cause the Alameda County Superior Court to issue an injunction. The Ralph Civil Rights Act, Civil Code section 51.7 "provides that all persons within this state have the right to be free from violence committed against themselves or their property because of their Â… position in a labor dispute."

A presentation by BAC Local 3 was eventually made on June 15, 2005, which included forecasts on economics and employment for the brick and construction industry of California. The projections indicate there will be approximately $60 billion in construction and 1,000 new bricklayers needed. Data sources included: the California Five Year Infrastructure Plan, published by the California Department of Finance; Project Labor Agreement data compiled from Construction and Building Trade Councils; various articles and publications that substantiate a surge in construction growth, such as a study from the University of the Pacific and various realty advisor groups; California E. D.D.; Labor Market Information Division; BLS; Construction Labor Research Council; Consumer Price Index (CPI) information; and a report from the California Budget Project on the cost of living in California.

The third party administrator of the BAC Health and Welfare trust fund reported the average monthly retiree benefit received by the bricklayers retiree is almost equal to the monthly contribution they have to pay for health insurance.

BAC Local 3 also presented the advantages of being a BAC Local 3 contractor. BAC Local 3 established a website (www. bac3-ca. org) with a section that promotes the contractors; purchased radio advertising to promote the trade, increase recruiting, and increase brand awareness; used direct mail to promote the contractors to general contractors, developers, architects and engineering firms; distributed publications that emphasize safer building practices using masonry; enhanced recruitment efforts through the Helmets to Hardhats program; participated in a television sponsorship on the history of bricks through the History Channel.

In July 2004 the bricklayers took a $1.50 per hour pay cut to fund their health and welfare benefit. BAC is asking for a raise to recover that pay cut and to cover future increases in health and welfare. Additionally, BAC is seeking an increase to cover the cost of living increase, such as gas and housing and a subsidy for the retiree's health and welfare.

As of today, the contractors association has not made a logical argument for their refusal to grant the bricklayers the requested increase in compensation.

Bricklayers and Allied Craftworkers represent approximately 3,000 Bricklayers/Stonemasons, Tile Layers and Finishers, Pointer Cleaner Caulkers, Marble Setters and Finishers, Terrazzo Mechanics and Finishers, and Refractory Bricklayers who work various aspects of the industry in the Northern 46 counties of California.

Sustained delivery of PDGF-BB directly at the wound site is believed to stimulate angiogenesis and granulation tissue formation through the recruitment and proliferation of cells such as monocytes, fibroblasts and endothelial cells, which are necessary for the stimulation of a variety of wound healing processes. A previous Phase 1/2 clinical trial showed that Excellarate appeared to be safe and well-tolerated and resulted in a high rate of complete wound closure.

GAM Technology

Gene Activated Matrix(TM) (GAM) technology is designed to provide a therapeutic level of protein synthesis at a particular site in the body and can be used in soft tissue such as skin, ligament, tendons and cartilage, as well as hard tissue such as bone. The technology is distinctive in that it is an immobilized form of local gene delivery that allows for control of gene dispersion. GAM consists of a biocompatible matrix comprising a gene or DNA vector encoding a growth factor or other therapeutic protein. For tissue repair, the application method involves placement of a GAM gel directly onto a wound site. TRC's studies have shown that proliferative cells in the body can migrate into the GAM, take up the immobilized vector and gene and then transiently express the encoded therapeutic protein. Compared with topical applications of proteins, this in situ expression method significantly prolongs the local availability of therapeutic protein to the cells involved in tissue repair. TRC's GAM technology may have potential utility in several clinical indications where protein therapeutics have had limited success, including treatment of dermal wounds (such as diabetic foot ulcers), therapeutic angiogenesis (pharmacologically inducing new blood vessel growth), and orthopedic products for repair of various tissues, including hard tissue (bone) and soft tissue (ligament, tendon, cartilage).
