The World Health Organization recently published new documentation that links radon gas to more deaths throughout the World than previously estimated. The Orion Health-Oracle and Fujitsu implementation at Son Dureta comprises three major, inter-related automation projects: a clinical web portal, a central repository for clinical data, and an electronic whiteboard for the accident and emergency department.

The team involved in the project included staff from the three companies: Fujitsu Services provided the design for system architecture, IT infrastructure, project management, monitoring and deployment of the solution.

Clinical Web Portal

Son Dureta chose Orion Health's Concerto as the clinical web portal through which clinicians can search and view information pertinent to their daily tasks, such as hospital intranet; existing web applications; and patient demographics.

The Concerto clinical web portal also provides access to Orion's web-based orders module, so that clinicians can also order tests electronically.

Electronic Whiteboard for Emergency Department

As part of the project, Orion Health has implemented an electronic whiteboard solution in Son Dureta's Emergency Department. The whiteboard system displays up-to-date information about the patients currently in the Emergency Department, including triage category, submitted orders, available lab results and summary demographics. These data are extracted from other information systems within the hospital and are updated every 15 seconds, providing an accurate "snapshot view" of current patient activity in the Emergency Department.

Centralised Repository for Clinical Data

The solution at Son Dureta uses Oracle Healthcare Transaction Base (HTB) as a centralised, standards-based repository for clinical data. Relevant data from the hospital's various existing systems is fed into Oracle HTB via Orion Health's Rhapsody Integration Engine, where it is normalised to allow for automated clinical decision support and important clinical comparisons. Clinical information stored and maintained in Oracle HTB at Son Dureta include clinical orders, lab test results and patient demographics.

Data stored in Oracle HTB is available for immediate recall and viewing by authorised clinicians via Orion Health's Concerto Medical Applications Portal. Concerto presents the HTB data in an on-screen format that is easy to read and act upon.

Dr. Mancini recently donated more than 1,200 books from his personal library to the university, understanding that "books are meant to be shared, not stored."

"I've always been an avid reader, and for more than 20 years I've saved the books that have influenced me most. I imagined one day I would have a home library and surround myself with these great books," said Dr. Mancini. "When I saw that UNEVE did not have many books, I felt inspired to share all this wisdom with their students for generations to come."

UNEVE is a public university located near Mexico City, offering Mexico's only chiropractic program. Dr. Mancini was instrumental in helping establish the program, which opened in 2001, and since then has produced hundreds of highly-trained and skilled chiropractors dedicated to bringing chiropractic services to the people of Mexico. Parker College is also active within the UNEVE community through a Clinic Abroad program that gives Parker students hands-on clinical experience in addition to Spanish classes and a total immersion experience in Mexican culture.

Dr. Mancini recently celebrated his 10th anniversary as president of Parker College of Chiropractic. “AIDS is a dangerous human affliction that must be fought with all tools at our disposal, and with the best collaborative efforts, not ridicule. No responsible government stays idle, while the people it swore to lead are stained and perished by a deadly virus or disease, meanwhile waiting for foreign assistance or cure to arrive. It must deploy every resources and special knowledge at its immediate disposal, with true and practical compassion for those infected. That is leadership,” Uba stated. “I am confident that the great people of South Africa will prevail, in their determination to defeat this menace.”

But success will require a new, brilliant approach that must deliver short and long-term solutions.

HIV virus tends to coat itself with sugar, a defensive psychology that makes it rather difficult to develop an effective cure for. And it has a tendency to, with a rapid climb, quickly lock the infected victim’s natural virus-fighting immune system gate. Thus, for a person with an already weakened or compromised immune mechanism, this means a rapid and consistent progression from HIV to AIDS, and subsequently death. This leads to African diet behavior. African daily nutritional behavior and value is critically and consistently lacking in fruits, fresh (not overcooked) vegetables and such well-known antioxidants as garlic. The result is a poor immune system. Therefore, it makes all the sense in the world that the strongest emphasis must be leveled on HIV testing platforms. Clearly, a very ambitious, nationalized HIV testing policy is long overdue for South Africa. This begs for coordinated private/public sector collaboration.

The South African government is clearly dispatched in its attempt, faced with limited resources, to balance the needs of the vast numbers of its citizens infected with this virus, one of the worst in the world, with the demands of protecting the rest of the population from further escalation. To meet this balance the government has taken to some policy guidelines that many AIDS advocates, among them the UN Special Envoy for AIDS in Africa, Mr. Stephen Lewis, call “immoral” and a product of the “lunatic fringe.” The government needs a much better reflection, representation and articulation of its AIDS policy to all South Africans and must endeavor to respond and speak to its infected citizens with broader compassion, not indifference. “Great nations and leaders are measured not by how they treat the strong and powerful, but by how they respond to the needs of the weakest members of the family.” “And the world must deplore, with grave disdain, any attempt to use this human crisis to undermine or discredit any government or country. Clear-headed dedication and proven solutions, not conflict making or unproven methods must prevail. This is a battle that must be won,” Uba concluded.

Collaborating with a specialized Genetic Center, special molecular probes were custom designed to identify the deadly gene sequence. By combining the probes with the embryos, the Center identified the abnormal embryos from the healthy ones. After two embryos were placed in her uterus, HK and Brian left overseas to begin their work as teachers.

Optimism began to build two weeks later when the pregnancy test was positive. However, caution prevailed until an amniocentesis was performed by removing fluid surrounding the baby at 4 months into the pregnancy. To the couple’s delight, they were blessed with a healthy baby girl, free of the deadly disease. Who May Benefit from PGD? Embryos that carry certain chromosomal or genetic conditions may fail to initiate implantation to the uterine lining. Other outcomes may be a pregnancy loss or the birth of a child with physical, mental, and learning disabilities. PGD is useful for patients who have other types of reproductive medical problems.

These are some examples of when PGD is recommended to patients:
Recurrent miscarriages (usually more than 2) Women 35 and older who produce 6 or more embryos with IVF Family history of chromosomal conditions or history of X-linked disease Unexplained infertility 2 or more unsuccessful cycles

It is well known that fertility in general decreases with maternal age. Research has confirmed that genetic abnormalities also increase with advancing maternal age. A major cause of low implantation is that many embryos are chromosomally abnormal and they seldom implant or get to deliver. PGD allows for the selection of the best quality embryos which are then transferred during IVF, therefore reducing the incidence of a chromosomally abnormal offspring.
