Janet Werner, a single Mom of a 12-year-old son, successful consultant to New Jersey’s pharmaceuticals and a former Rutgers University counselor speaks up on why the message, not the label, of ‘Indigo’ is so important.

According to P. M. H. Atwater, author of Beyond the Indigo Children, they are unusually intelligent (even if they are flunking in school and can't spell), gifted entrepreneurs, exceptionally creativity, intuitive, have strong spatial reasoning skills (both sexes) and are volunteer-minded and group-oriented.

Ms. Werner continues, “Atwater calls them ‘quick-click kids’ who expect things to come to them with little waiting for slow outmoded systems to catch up. This is why the topic is so important.”

She contends that when youth get frustrated and blocked in their desire to connect and communicate within the maze of our traditional thinking, they withdraw and become depressed. Our response as adults is often to drug, suppress and label them as our quick fix.

Atwater notes this generation contends with a dizzying array of learning disorders that have confounded psychologists and counselors. College campuses have seen a drastic increase in issues relating to student mental health in the past decade. The American College Health Association 2004 survey noted psychiatric medications rose 24.5% in 2003-2004 from 17% in 2000, and just 9% in 1994.

In 2004 The New York Times quoted Dr. Mark Harris, University of Iowa, saying his counseling center saw 20% more students in September of 2004 than 2003, “The worst fall for emergencies in two decades,” stated Dr. Harris.

Mr. Scott Fritz of Red Bank, New Jersey, sees the issue even more drastically from the point of view of a parent who lost his child, Stephanie, to suicide two years ago. “Suicide is the third leading cause of death for those under age 24. In New Jersey alone we lost 74 kids in 2004. These deaths significantly affected the lives of thousands in our communities.”

Two weeks ago, Mr. Fritz and his co-advocate Don Quigley in partnership with Senator Ellen Karcher realized the signing by former Governor Codey of the New Jersey Suicide Prevention Law, which mandates formal teacher training on suicide prevention in secondary schools.

Gregory Roberts Ed. S., Executive Director American College Personnel Association (ACPA) in Washington DC, approached Ms. Werner and asked her, “How do we address the mental, emotional, physical, social and spiritual health of our college students?” He noted he is finding a thirst for spirituality from students.

“Adults interpret this acute intuition combined with an almost built-in resistance to authority as disruptive. But we could be describing the best & the brightest from the 60’s & 70’s – our baby boomer mavericks who I worked extensively with in the corporations,” asserts Ms. Werner.

“Our kids today don’t want to be labeled, they don’t want to talk about their talents – they want to do something, they have no tolerance for our discussions of labels. I know because in 1979 I was labeled and medicated myself,” shares Ms. Werner. “Indigo after all is just a color. Let’s rethink how we think in our conversations. Lets join with them to change the status quo and open up conversations using The Indigo Evolution’s message. It is unacceptable that kids seek suicide as the solution.”

1. Remember you are never a passive observer to your own safety. Your safety is a shared responsibility between you and emergency services authorities including police, security, and others. Pay attention to your surroundings including fire exits, bottlenecks in crowds, announcements and general conditions. The police and security staff cannot be everywhere at once. You must be the first line of defense; be alert, aware and proactive.

2. Don't run from danger, run toward safety. While this is counterintuitive, running from danger, especially with a crowd or mob trying to escape, may very well put you at greater risk. Running toward safety advance planning and awareness. Mentally rehearse escape routes or survival behaviors before something actually happens.

3. File a "Flight Plan". Just as a pilot plans and documents a route, let others know where you will be throughout the day and how you expect to travel from place to place. Whether it's a local drive or an overseas trip, share your approximate itinerary with someone. If they hear about a problem on radio or TV, they may be able to warn you. If you're in a jam and your family and friends know where you are expected to be, they can mobilize assistance if you don't arrive as scheduled. Create a communications plan ahead of time.

4. Know Before You Go. Civil unrest, military conflicts and health risks such as disease outbreaks can surface suddenly and turn a vacation into nightmare. The U. S. State Department, and other sources, publishes travel advisories and warnings in real time.

Visit www. state. gov or www. firstgov. gov and consider bookmarking them on your mobile phone or e-mail.

5. Trust your instincts. If something feels wrong, it probably is. If you feel uncomfortable in your travels, give yourself permission to leave. Don't worry about how you will be perceived. You can always return later, on your terms, when you feel safe. Don't ignore your internal security system.

From nation to nation, the sacred eagle feather will pass hand to hand, giving Native youth the responsibility to lead us into a brighter future. On May 17th, Berkshire residents get the chance to meet, share a meal, and host thirty Native runners in their homes overnight from the Wolakota Prayer Run for World Peace visiting Great Barrington on its cross-country trek. Our Sacred Future Foundation is co-sponsoring a public pot-luck event with Berkshire South Community Center to welcome the Youth Runners on Tuesday, May 17th at 5:30pm, located at 15 Crissy Road in Great Barrington, MA.

Runners from American Indian reservations nationwide begin their journey from the Four Directions (Canada, Mexico-California, New York & North Carolina) in early May. Ending on June 18th in Peidmont, South Dakota aka Paha Sapa "Black Hills" - sacred site of the Lakota/Dakota/Nakota nation.

They will gather with over 10,000 people for the 10th annual World Peace & Prayer Day, founded by Chief Arvol Looking Horse. The Run is open to non-Indigenous youth and adults as well.

The run is a rite of passage for the youth, where they learn to use traditional knowledge, connect with the Earth through prayer, and to experience first hand waking up with the sun, visiting different communities, and sharing their prayers for peace on the road. Sky Oldshield (Seneca/Dakota), the Wolakota Youth Council's Advisor says, "The greatest enemy is our self. We have to be a new kind of warrior, battling with our hearts and minds. What we face today calls for action from our hearts; otherwise, there will be no future for our youth. We must remember the sacred teachings of our ancestors, that we are all one."

The Wolakota Youth Council is sponsored by local Berkshire residents Melissa Kushi and Chris Lindstrom, co-founders of the non-profit Our Sacred Future Foundation, which is dedicated to renewing the wisdom and essence of traditional Native foods, agriculture, and healthy sustainable living. Their projects are devoted to education for sustainability, appropriate technology, alternative currency, and building sustainable community by bridging traditional wisdom with eco-technology.

Hymotion and A123Systems today announced that the South Coast Air Quality Management District (AQMD) has awarded the companies a contract to provide 10 converted Priuses to plug-in hybrid electric vehicles (PHEVs). The South Coast AQMD PHEV program will evaluate and help identify a path to commercialization in California.

The AQMD has identified the use of near-zero emission vehicle technology as a key air quality attainment strategy, and has sponsored PHEV demonstrations for over six years because of the potential for this technology to enable zero-tailpipe emissions for portions of a typical driving cycle. Similar to commercially available hybrid-electric vehicles (HEVs), PHEVs utilize a battery pack and an electric motor in concert with an internal combustion engine. PHEVs, however, employ a larger battery pack which can be designed to extend the electric portion of the driving cycle, providing improved fuel economy, lower greenhouse gas emissions and reduced petroleum dependence.

The Hymotion solution incorporates A123Systems' batteries into a highly advanced PHEV module that is lightweight, compact and requires minimal modification to the stock vehicle. All necessary components and safety features are integrated and contained within the module, including: batteries, power electronics, crash sensors, power electronics, charger, battery management system, safety sensors and manual-electric interlock. Due to its plug - and play - installation, the system does not require removal of the OEM battery pack and can be installed in less than 2 hours.

The awarded solution uses A123Systems' nanophosphate technology that provides unprecedented specific power, safety and life - all critical to the optimization and commercialization of PHEVs. The system is expected to maximize fuel economy while maintaining California's most stringent vehicle tailpipe emission standards. Early demonstrations have shown greater than 100 miles per gallon in city driving, which cuts carbon dioxide emissions in half. The solution also includes power processing and rapid chargers provided by AeroVironment, Inc.

The American Parkinson Disease Association (APDA), www. apdaparkinson. org, has announced its 2007-2008 fiscal year research awards, which total more than $3.5 million and include its ninth Center for Advanced Research, creation of a $100,000 contingency fund for research to assure projects in progress from being lost to institution budget cuts, and the establishment of a dedicated post-graduate geriatric psychiatry grant. It also increased its Centers for Advanced Research allocation from $100,000 a year for five years to $125,000 annually for the same period. The awards bring APDA’s total Parkinson’s disease research contribution to more than $30 million since its inception.

The University of Chicago was chosen to join the schools of medicine at Emory University, Atlanta, Ga.; UMDNJ-Robert Wood Johnson, New Brunswick, N. J.; Boston University, Boston, Mass.; and UCLA, Los Angeles, Cal.; the medical centers at the University of Virginia, Charlottesville, and Washington University, St. Louis, Mo.; and the Universities of Pittsburgh, Pa., and Alabama at Birmingham, as Centers for Advanced Research. Un Jung Kang, MD, is the new advanced center’s director.

Two Dr. George C. Cotzias Memorial Fellowships, an $80,000 grant for each of three years, were awarded to Talene Yacoubian, MD, PhD at the University of Alabama at Birmingham, and Bradley Miller, MD, PhD at the University of Virginia. The fellowship is named for George C. Cotzias, MD, a pathfinder in pharmacological exploration of brain functions and in the treatment of Parkinson’s disease with levodopa, which more than 40 years later is still the gold standard of PD medication therapy.

Princeton University PhD, Jeffry Stock, and Andrei Alexandrescu, PhD, of the University of Connecticut, Storrs, were named Roger C. Duvoisin, MD grant recipients, an award of $80,000 for two consecutive years.

APDA’s Scientific Advisory Board, which reviews and recommends all research allocations to the APDA board of directors, also chose 31 research grants and 11 post-graduate grants and four summer medical student fellowships.

Researchers at 24 institutions across the country received one-time $50,000 research grants including two at Georgetown University, Washington, D. C.; Johns Hopkins University School of Medicine, Baltimore, Md.; the University of Texas Health Science Center at San Antonio, Tex.; Columbia University, New York, N. Y.; UCLA, Los Angeles, Cal.; and three at the University of Alabama at Birmingham.

One-year, $35,000 post-doctoral fellowships was awarded to two scientists at Massachusetts General Hospital, Boston, and the University of Alabama at Birmingham, as well as researchers at the University of Rochester, N. Y.; Beth Israel Deaconess Medical Center and Brigham and Women Hospital, Boston; Whitehead Institute for Biomedical Research, Cambridge, Mass.; Columbia University, the University of Virginia and Washington University.

APDA was founded in 1961 with the dual mission to “Ease the Burden - Find the Cure” for Parkinson’s disease and has grown into the country’s largest grassroots organization serving persons with PD and their caregivers. In addition to its $30 million research funding, APDA has contributed more than $28 million to patient and caregiver support through its network of 62 Information & Referral Centers and 800 support groups across the country.

As reported in a December 20 Newsweek Web Exclusive, the holiday season is often a challenging time for drinkers. But new options, including anti-craving medication and online support, are now available. Specifically, the web-based forum www. mywayout. org was recommended. Alcoholics Anonymous was also suggested.

Roberta Jewell, author of the self-help recovery book, My Way Out, serves as administrator of the Internet support group at www. mywayout. org/community (http://www. mywayout. org/community) and says she sees a bump in traffic -- and relapse -- at the end of each year.

"A lot of people are struggling now," says Jewell. "they've often held the bottle at bay for a long time, but craving is a condition many individuals live with on a day to day basis. Holiday pressures can add to the stressors that serve as a trigger. Depression sometimes hits, too. So it's reassuring to know you can reach out from the comfort of your home, anytime, and find help."

Traffic at My Way Out's free forum is nearly four times that of the Alcoholics Anonymous website (source: www. alexa. com). But Jewell says many of her members attend local 12-step meetings and the two are not mutually exclusive.

"Still, the majority of individuals who visit us online are simply uncomfortable in a face-to-face environment, at least early on in their recovery," she says. "So they seek out a more private venue which is truly anonymous and in which they feel comfortable sharing information about their situation. It can be very therapeutic."

Dr. Linda Garcia, MD, addiction specialist and Medical Director of My Way Out, agrees that participation in a web forum can be immensely helpful. She says abstainers fall off the wagon disproportionally around the holidays and are sometimes frustrated by their inability to get in to see a doctor or therapist.

"It's important to prepare," she says. "There tends to be an increased workload for many physicians because they are dealing with more depression, suicide, coronary heart problems and other conditions now. Appointments are also hard to get because patients are trying to satisfy end of year deductibles."

To prevent relapsing Garcia recommends adopting good nutrition with limited sugar intake, amino acid supplementation in addition to vitamins and minerals, a regular exercise program, and some form of relaxation. She says anti-craving medication, hypnotherapy and counseling are also important options to consider.

Jewell agrees. She says developing a customized recovery program goes hand in hand with the support visitors find once they arrive at the My Way Out forum.

"It's a multi-faceted approach," says Jewell. "Support is a critical piece. We're able to provide excellent resources to help people work with their physicians and develop a plan that's appropriate. They also find friendship and fellowship along the way. We've seen a lot of success stories and we're really proud of that."

The new Centegra Gavers Breast Center in Crystal Lake, currently the Crystal Lake Imaging Center, will serve as the hub for Centegra Health System’s Breast Health program. When it opens in the fall, it will provide McHenry County patients identified with abnormal screening mammograms a full continuum of follow-up diagnostics, required care and social and emotional support in a coordinated and centralized location. Centegra has several state-of-the art imaging facilities throughout McHenry County that provide convenient access to individual imaging needs. Women will be able to get the latest digital screening mammography close to home and in the event more detailed evaluation and treatment is needed, the Centegra Gavers Breast Center will be a dedicated breast care center they can rely on. Imaging services are also offered at Centegra Hospital-McHenry, Centegra Hospital-Woodstock and Centegra Health Center-Huntley.

“The Gavers Community Cancer Foundation has generously committed to donating substantial funds to make the Centegra Gavers Breast Center a reality,” said Centegra President and Chief Executive Officer Michael S. Eesley. “The Gavers Community Cancer Foundation has had a long-standing commitment to increasing cancer awareness and services at Centegra, and we are proud of this partnership that will propel Centegra to the forefront of breast care treatment in our region.”

“A dedicated Breast Center is an important venue in improving breast cancer patient outcomes,” said Schwaab. “By also working toward a three-year accreditation with the American College of Surgeons, Centegra will be in a stronger position to serve more patients, meet identified community needs and gain patient and physician recognition as an accredited provider of high quality breast care.”

Compagnoni said, “Access for specialized diagnostic and consultative services for McHenry County women facing breast cancer will be coordinated in a geographically-centralized location in Crystal Lake. Screening services will continue to be offered at various locations throughout the county, while the Breast Center will serve as a hub of the program as well as a face for coordinated breast health services in the community.”

Radiation therapy, infusion therapy and additional support services will continue to be offered at the Centegra Sage Cancer Center in McHenry.

In 2008, Centegra Health System submitted a Certificate of Need with the State of Illinois to build a dedicated women’s center in Woodstock. An economic downturn has put that project on hold, however, a coordinated and dedicated Breast Center was one of the women’s programs and services identified in that Certificate of Need application.

Centegra Health System and the Gavers Community Cancer Foundation also partner year-round to raise awareness of the importance of early cancer detection. The Get Checked! Program provides information about lifestyle changes and guidelines for routine screenings that can make a big difference in one’s life. The Gavers Cancer Resource Library is also featured in Centegra Sage Cancer Center in McHenry and offers touch-screen computers, a library inventory of books, journals and booklets. With sales in 2003 estimated at $17.5 billion, the U. S. market for over-the-counter (OTC) drugs is by far the largest in the world, but it is arguably far from being the most dynamic self-medication market. The U. S. markets sluggish performance is attributable in part to the very cautious attitude of the FDA toward the reclassification of prescription drugs as nonprescription medicines, a process known as "Rx-to-OTC switching." However, recent statements made by the FDA indicate that its attitude toward Rx-to-OTC switching is about to change.

We begin this report by considering the perspectives on self-medication of three major stakeholder groups: consumers, payers, and manufacturers, plus the position of the FDA. We then examine the cost-containment implications of Rx-to-OTC switching in two drug classes that have had recent switch approvals (nonsedating antihistamines and proton pump inhibitors) and two classes that may see switches in the near future (statins and obesity therapies). We conclude with a brief assessment of the outlook and implications for the pharmaceutical industry.

Business Implications

Although the default status of medicines in the United States is nonprescription, the Food and Drug Administration (FDA) has traditionally been very cautious in regard to Rx-to - OTC switching (i. e., the reclassification of prescription drugs as nonprescription medicines). Far fewer compounds are available in OTC form in the United States than in the other major self-medication markets. However, the FDA has stated that it intends “to become more proactive in recommending key potential switches."

Given that prescription drug prices are generally significantly higher than the prices of comparable OTC medicines, manufacturers usually have a vested interest in maintaining the prescription status of their new chemical entities as long as possible. However, as a drug approaches patent expiration and the threat of generics competition, it may be advantageous for the manufacturer to switch the drug to OTC status as a way of extending its life cycle. The timing of the switch is crucial: it should be early enough to develop consumer loyalty to the OTC brand but late enough to exploit the full sales potential of prescription sales.

When a drug is switched to OTC status, health plans often end reimbursement of other drugs within the same class or move these drugs to a higher formulary tier that incurs copayments higher than the OTC drug’s retail price. On average, copayments for third-tier drugs (i. e., nonpreferred brands) exceed $30 per month. The substantially lower prices of OTC drugs compared with competing prescription drugs have persuaded many health plans to add certain switched drugs to their formularies, often in a new tier that is subject to a very modest copayment (e. g., $5). According to a recent managed care survey, 31.5% of HMOs covered at least some OTC medicines in 2003.

Recent FDA judgments suggest that the pharmaceutical industry may need to be somewhat bolder in its dosing proposals in switch applications. In the past, manufacturers have assumed that reducing the dosing from standard prescription levels would enhance their prospects of succeeding with their switch applications, but the opposite has been the case in some instances. The FDA has shown that it is unwilling to approve switches that may use subtherapeutic dosing.
