Nearly $30 million a year would be saved in hospital charges if early rather than delayed ACL (anterior cruciate ligament) reconstruction surgery was performed on pediatric patients, according to a study presented today at the American Orthopaedic Society for Sports Medicine’s (AOSSM) Annual Meeting in Providence, Rhode Island.

Nearly $30 million a year would be saved in hospital charges if early rather than delayed ACL (anterior cruciate ligament) reconstruction surgery was performed on pediatric patients, according to a study presented today at the American Orthopaedic Society for Sports Medicine’s (AOSSM) Annual Meeting in Providence, Rhode Island. Additionally, more than 7,300 tears to the meniscus and 7,800 cartilage tears in children could be avoided each year in the U. S. by early ACL surgery.

“The timing of pediatric and adolescent ACL surgery has historically been controversial,” said Theodore J. Ganley, MD, Director of Sports Medicine and Associate Professor at the Children’s Hospital of Philadelphia. “The theoretical risks of growth disturbance in younger patients are balanced against the risk of further knee damage related to delaying treatment until closer to skeletal maturity.”

The goal of ACL knee surgery is to stabilize the knee allowing patients get back to a healthy, active lifestyle. Long-term, the surgery aims to prevent instability and additional damage to the knee.

A 14 year review of ACL reconstructions presented by the authors at the 2009 AOSSM Annual Meeting revealed a 4 to 11-fold increase in meniscal and cartilage injuries with a greater than 12 week delay in ACL treatment.

In the current study, a model for pediatric ACL reconstruction was developed based on probabilities derived from the ACL review. Identical groups of 100,000 patients, representative of the U. S. population were simulated to undergo either early or delayed ACL reconstruction, with the secondary meniscal and cartilage damage and hospital charges compared between the two groups.

“The decision tree and statistical modeling approach for the study created by my co-author Suneel Bhat, is unique in that it incorporates variability, thereby generating a model simulation of a large scale prospective study, which provides a way to generalize implications,” said Dr. Ganley.

The simulation found that in females in the U. S., delaying ACL reconstruction beyond 12 weeks resulted in 1,560 medial meniscal tears and 2,100 cartilage tears relative to early surgery each year. In males, delayed surgery resulted in 3,300 medial meniscal tears and 5,720 cartilage tears relative to early surgery.

The study revealed that more than $29.4 million would be saved in hospital charges for pediatric patients each year in the U. S. by reconstructing ACL tears early rather than delaying treatment.

Similar to recent actions taken by the FDA (http://www. fda. gov/Drugs/GuidanceComplianceRegulatoryInformation/EnforcementActivitiesbyFDA/WarningLettersandNoticeofViolationLetterstoPharmaceuticalCompanies/UCM055773), this warning letter has been widely reported on a range of news sites and blogs and has lit up robust discussion in social media and topic-specific channels. While FDA’s move should come as no surprise to those following the evolving regulatory environment for social and other digital media, it is disappointing nonetheless.

Not only does it suggest that regulators lack an understanding of how consumers use the Internet to find and act on health information, it reveals a continued tactic-by-tactic approach to enforcing pharmaceutical marketing regulations. FDA’s continued application of old broadcast tactics and rules to narrowcast environments, and limiting that application to one set of players in those environments, puts consumers increasingly at risk from misleading and false treatment information. If you think you know where you will be tomorrow, perhaps you should rethink the horrific consequences of that reasoning. We all die. Be it the result of age related natural causes or at the hand of some unforeseen disaster, your time is coming and fate most certainly will deal its hand.

The sad reality is that somewhere between 50 and 70 percent of Americans do not even leave a Simple Will. When it comes to a complete estate plan nearly 98% of use fail to reach all the legal requirements needed for our asset protection efforts to stand up in court.

While a simple will may sufficiently serve the estate planning needs of many people, estate planning is highly complex. You should always meet with a qualified legal advisor to be sure the estate plan you have in mind is in reality, consistent with your objectives.

When you approach an interview, it is with the intent to obtain perhaps the job of your dreams. You put your best foot forward. You go into the interview with the intention to be the best choice decision that company ever made. You accomplish every goal and learn every detail and requirement it takes to be promoted. You do what it takes to increase your worthiness, importance and retention. You do this for the sake of wealth building and future financial security.

The same priorities must be accomplished in your personal estate planning needs. When you take the time to prepare the legal documentation necessary to protect your assets and heirs, you are the one who stipulates how your property is distributed and who raises your minor children when you are gone. If you die intestate (without estate planning), the probate court intervenes and a judge (tasked to interpret the black and white letter of the law) decides how your assets are divided. And if you are a single parent of minor children, that same judge will decide with whom your children will live.

No one says that the legal process is not complex and ever changing. With wills alone there is the simple will, living will, testamentary trust will, pour-over will, last will and testament, etc. There are various trusts, proxy’s, vulnerable assets, long term health care initiatives, taxes, inheritance or probate advances, etc. to be considered essential to financial security and estate planning.

A point to acknowledge is you must not only recognize that estate planning although complex is also an ever changing (on-going) process. It requires periodic reviews based upon your changing goals and financial complexities. Because there are so many things relative to proper estate planning, it often involves the coordinated effort between qualified legal, tax, insurance and financial professionals.

Do not confuse the drafting of legal documentation with the development of an estate plan. You also must realize it is important to have everything accessible in one location.

Once you have your will, durable power of attorney, health care proxy and other legal documentation Additionally, you need other key data. This information includes at a minimum, Social Security numbers, financial account numbers, , health insurance, Logon/passwords for your computer, original marriage certificate, divorce decrees, military discharge paperwork, life insurance information, deeds and appraisals, beneficiary designation forms, auto and home insurance policies, a listing of important contacts with address and phone numbers, bank and safety deposit box locations, lawyer, doctor, accountant and other professional points of contact.

You need to decide how your beneficiaries will find your essential documents. Who will have access to these documents and when that access is permissible. You should consider storing this vital documentation securely in a place other than your home.

Asset protection and estate planning is a process of understanding your legal initiatives, alternatives and obligations. It is a process of accomplishing the needed tasks to protect your most important information that guides and protects your heirs. It is a process that needs acted upon immediately and demands periodic review and initiation of needed revisions.

El día de hoy, el Subsecretario para América del Norte, Gerónimo Gutiérrez Fernández, realizó una visita de trabajo a Los Ángeles con objeto de participar en el lanzamiento de la segunda etapa del programa de la Ventanilla de Salud del Consulado General de México en Los Angeles. En este evento estuvo acompañado por el Director del Instituto de los Mexicanos en el Exterior, Cándido Morales, y por el titular de esta representación, Rubén Beltrán.

La Ventanilla de Salud inició sus trabajos en dicha representación consular en marzo de 2003. Los componentes esenciales de la Ventanilla son: educación preventiva; información sobre seguros de salud; referencias a clínicas y servicios de salud de bajo costo, así como servicios proporcionados por clínicas médicas móviles en las instalaciones del Consulado.

Durante este evento, el Subsecretario Gutiérrez refrendó el compromiso del Gobierno del Presidente Vicente Fox por mejorar las condiciones de los mexicanos que viven en Estados Unidos. Además, manifestó que se tienen instrucciones presidenciales muy explícitas de trabajar en pro de la defensa de los derechos de los mexicanos que se encuentran fuera de territorio nacional y de promover el mejoramiento de sus condiciones de vida.

Con base en dichas instrucciones, el Subsecretario para América del Norte encomió el trabajo que se ha venido desarrollando durante los dos últimos años en la Ventanilla de Salud del Consulado General de México en Los Ángeles y aseguró que se continuará apoyando este proyecto tan importante que ya se está replicando en otros consulados mexicanos.

En ese sentido, y con el objetivo de coordinar los esfuerzos de expansión del proyecto de Ventanilla de Salud a otros consulados, se nombró recientemente a Sonia Contreras como Directora de las Ventanillas de Salud en Estados Unidos por parte del IME.

En el mismo sentido, el Subsecretario Gutiérrez destacó que durante la primera etapa de este proyecto, se tuvo un impacto muy positivo que benefició a más de 150 mil personas otorgándoles entre otros servicios: sesiones educativas de salud; información sobre seguros médicos o atención por parte de alguna de las clínicas móviles que visitan el Consulado General todos los lunes.

Finalmente, el Subsecretario Gutiérrez enfatizó que gracias al éxito de la Ventanilla de Salud de Los Ángeles se logró continuar con el importante apoyo económico de “The California Endowment”, que, a la fecha, ha destinado recursos por un millón de dólares para este programa en particular.

Por su parte, el Cónsul General Rubén Beltrán agradeció a todos los socios que participan en la Ventanilla de Salud de Los Ángeles, como la agencia fiscal para el proyecto “Neighborhood Legal Services of Los Angeles County“ (NLS), encabezado por la Consejera del IME, Yvonne Mariajimenez; Maternal and Child Health Access; el Departamento de Servicios de Salud del Condado de Los Ángeles (DHS); ALTA – MED; Health Net, y el Proyecto del Barrio.

Como parte de la agenda de trabajo en esta ciudad, el Subsecretario Gerónimo Gutiérrez también sostuvo encuentros con Antonio Villaraigosa, Alcalde electo de la ciudad de Los Ángeles; Ann Marie Tallman, Presidenta y Consejera General del Fondo México–Americano para la Defensa Legal y Educativa (MALDEF, por sus siglas en inglés); miembros de la junta editorial del periódico La Opinión y líderes comunitarios.
