Valencia, California, Plastic Surgeon Weighs in on Silicone Breast Implants – 3 Years After FDA Approval

Nearly three years ago, the U. S. Food and Drug Administration approved the use of silicone implants for breast augmentation procedures in women over the age of 22, and breast reconstruction procedures for patients of any age. The FDA’s decision capped a controversy that spanned more than a decade, and one plastic surgeon in Valencia, California says, while silicone implants are safe, there are still disadvantages.

In 1991, the FDA pulled silicone breast implants off the market for further study of their safety. Over the next 14 years, the implants would become the subjects of intense research, clinical trials, and fierce controversy. In 2006, the FDA approved them for use, and plastic surgeons across the country have experienced an uptick in procedures involving silicone breast implants, as opposed to their saline counterparts.

Dr. Marc Lussier is the medical director of Town Center Surgery, a practice in Valencia, California offering plastic surgery to Bakersfield and the surrounding region. He says, although the ratio between silicone and saline breast implant procedures he performs is about even, the demand for silicone implants at his Valencia Bakersfield area breast augmentation practice is increasing – possibly due to the fact that silicone offers a softer, more natural feel and appearance. “Saline is more artificial-looking, not as soft. I tend to use saline in younger women who haven’t had children. I use more silicone in women who have had children and/or need breast lifts.”

Dr. Lussier says he believes silicone breast implants are safe; however, there are some disadvantages. “In terms of systemic disease, ‘silicone breast implants’ are very safe. In terms of local complications, there is a higher incidence of capsular contracture, more frequent operations, more costs. With saline implants, patients are more comfortable with this product due to the bad press silicone has had over the years.”

Capsular contracture is the formation of scar tissue around the implant, resulting in a hardened feel and appearance. The condition can be treated with surgery. Dr. Lussier performs a number of different plastic surgery procedures, including breast augmentation, breast implant revision and he says, with any surgical procedure, there are risks of complications – and that includes breast implants of both types. Dr. Lussier says the best thing for patients to do is to have thorough and detailed conversations with their plastic surgeon, exploring all aspects of procedures and getting all of their questions answered so they can make the most educated decision about what’s right for them.

In terms of preference, Dr. Lussier says he has treated patients who have had revision breast augmentation surgery and replaced their original saline breast implants with silicone, which gives them a unique perspective to be able to compare both types of implants. “They much prefer the silicone if there are no problems with capsular contracture.”

Dr. Lussier says the main problem he’s seen with silicone implants is when to exchange them. He says patients should pay attention to the implant manufacturer’s warranty and also follow up periodically with their plastic surgeon. “I usually advise patients to get an MRI at 9 years because this is within the warranty period of the manufacturer. They should plan to exchange them in 10-12 years ‘after implantation,’ pending more information ‘from the FDA’.”

Overall, Dr. Lussier says that silicone breast implants get a satisfactory response from his patients, particularly those who have experienced both silicone and saline implants. But Dr. Lussier says there are still disadvantages to silicone, and patients should always weigh those along with the potential benefits. “’Silicone implants’ are an excellent and perhaps better option, but they are more costly, and have a higher maintenance cost. In automotive terms, it is the difference between a nice car and a sexy sports car. The latter is more expensive to drive, but it’s more fun.”

While more than one-third of home health care and nursing home organizations said they expect the government's economic stimulus package to increase use of healthcare information technology, another 52% said the stimulus package will have little or no impact on their businesses, according to a recent survey by IVANS, Inc. "Home health and nursing home care is expected to be one of the fastest growing provider segments over the next several years," said Clare DeNicola, IVANS President and CEO. "IVANS analysis reveals that while providers are optimistic about reform's ability to improve patient outcomes, budget concerns, understaffing, and the lack of clarity in the American Recovery and Reinvestment Act regarding stimulus funding for long-term care has them concerned."

Providers Looking to Electronic Health Records to Improve Patient Care
According to the survey, nearly 70% of home health care and nursing home organizations say that electronic health records (EHRs) will have a positive impact on their businesses, and 56% of respondents have begun or plan to implement EHRs within the next year. Other key technology priorities for long-term care facilities include implementation of wireless networks, solutions to provide electronic documentation at point of care, and business continuity planning, the survey shows.

When it comes to sharing data outside the organizations, survey respondents were not as positive. More than 70% of respondents said they have no plans to participate in a Health Information Exchange (HIE). Experience with failed efforts in the past, lack of an immediate driver to move forward, and no standards were all noted as barriers to implementation of HIEs.

The survey, conducted by IVANS among 500 healthcare providers in May 2009, drew responses from more than 300 home health care and nursing home organizations, whose specific survey responses are reflected in this release. In addition, IVANS conducted in-depth interviews with selected long-term care organizations. It's been a year almost to the day, but NASA researcher Dr. Steve Goodman still hasn't forgotten May 3, 1999.

On that date, more than 50 tornadoes cut a killer swath across the Great Plains of Kansas and Oklahoma. Property damage was estimated at $1.2 billion. More than 40 people died.

In hope of avoiding another May 3, 1999, Goodman and other scientists at the Global Hydrology and Climate Center, managed by NASA's Marshall Space Flight Center in Huntsville, Ala., are studying new methods of predicting severe storms.

And they believe another dangerous element of severe weather may be the key.

Using a combination of ground and space-based weather monitoring equipment, Goodman and colleagues at the National Oceanic and Atmospheric Administration in Washington, D. C., and at MIT Lincoln Laboratories in Lexington, Mass., have documented nearly a dozen cases in which lightning rates increased dramatically as tornadic storms developed.

"Our studies show a very big spike in the lightning's flash rate prior to formation of a tornado," Goodman says. "It's an early clue for weather forecasters to take a more detailed look at other storm characteristics with radar. And perhaps a chance for them to get warnings out earlier, saving more lives."

Goodman's team will present its research to scientists, meteorologists and emergency management officials from around the country at the "National Symposium on the Great Plains Tornado Outbreak of 3 May 1999," which opens April 30 at the Westin Hotel and Resort in Oklahoma City.

Spotting the telltale lightning flashes isn't as easy as keeping an eye on the sky from your front porch. According to Goodman, the type of lightning NASA is researching occurs within clouds, invisible to the naked eye by day. To properly monitor this type of lightning takes special equipment like NASA's Lightning Imaging Sensor, an instrument flying aboard the Tropical Rainfall Measuring Mission (TRMM) satellite launched in 1997. The sensor tracks worldwide lightning strikes and their relationship to storm centers.

Theories linking in-cloud lightning and tornadic storms have been debated for many years, according to Goodman. For decades, meteorologists and scientists pondered the connection. "But they lacked the ability to properly document and map in-cloud lightning," he says. "With the technological advances we've made in recent years, we can see what they couldn't."

Goodman is realistic about the work that remains ahead. "We don't have enough data yet to say how often the high flash rate precedes tornado formation," Goodman says. "But looking at this lightning signature can help pinpoint storms that are likely candidates, and that can make a big difference."

That difference would provide earlier warnings to increase citizens' chance of reaching shelter, and would likely reduce the number of false alarms that go out every year.

"Lead time for tornado warnings is better than it's ever been," Goodman says. "It's gone from eight to 12 minutes nationally. But the false-alarm rate hasn't changed. Only 30 percent of rotating storms ever make a tornado. That leads to a lot of false alarms lulling the public into ignoring the threat."

"It's a question of accuracy," he adds. "The more accurate we are, the more people take the proper response. That's what this research is all about."

Goodman's presentation, "May 3 Tornadic Supercells Viewed from Space During an Overpass of the NASA TRMM Observatory," will be May 1 at 4:10 p. m. CDT. The Great Plains Symposium runs through Wednesday, May 3 -- the anniversary of last year's devastating tornado outbreak.

The Global Hydrology and Climate Center is a joint venture between government and academia to study the global water cycle and its effect on Earth's climate. Funded by NASA and its academic partners and jointly operated by NASA's Marshall Space Flight Center in Huntsville, Ala., and the University of Alabama in Huntsville, the Center conducts research in a number of critical areas. For the first time ever, a group of high-level scientists assembled for the purpose of inventing something as close as they could get to the long-sought nanotechnology goal of building precise products atom by atom. The remarkably advanced projects those scientists produced -- which they hope to complete in three to five few years -- suggest that the era of molecular manufacturing could arrive far more swiftly than previously imagined.

"What this shows, even more strongly than before, is the critical necessity of additional work on implications and policy," said Mike Treder, Executive Director of the Center for Responsible Nanotechnology (CRN). "Existing nanotechnology policies, and most proposed policies, do not address huge new areas of concern raised by tomorrow's revolutionary manufacturing potential. That gap could be calamitous."

Nanofactories will use vast arrays of tiny machines to fasten single molecules together quickly and precisely, allowing engineers, designers and potentially anyone else to make powerful products at the touch of a button. In a single week of intense interdisciplinary work, an "IDEAS Factory on the Software Control of Matter" produced three ground-breaking research proposals that bring the nanofactory concept closer to reality. The project was sponsored by the UK's Engineering and Physical Sciences Research Council (EPSRC), a national science agency that also will fund the proposals.

"If, as expected, nanofactories can be used to build more nanofactories, then the impacts on society may be extreme," said Treder. "From remarkable advances in health care, environmental repair and poverty reduction, to severe economic disruption, political upheaval and the possibility of a new arms race: all these implications and more must be understood. Now it appears that our time to prepare is getting shorter."

The goals of the IDEAS Factory project were audacious: to make progress toward the vision of a "matter compiler" that could build atomically precise products under computer control. The forward-looking proposals coming from the IDEAS Factory should expand expectations as to what's possible at the nanoscale, and hold the potential to accelerate the development of nanofactory systems.

"This shows that molecular manufacturing, which has been considered a far-future result of nanotechnology, is now a fruitful topic for current scientific attention," said CRN Director of Research Chris Phoenix. "We expect that the IDEAS Factory will be a trend leader, inducing other nanoscientists to use molecular manufacturing as an inspiration and target for their work."

Participants in the IDEAS Factory designed research projects using an innovative process in which scientists from many different fields work together to bypass the conventional limitations of their fields. The three proposals they developed are expected to accomplish in just a few years what might have taken twenty with traditional approaches. Funding has already been assured by the EPSRC and experimental work will begin shortly.

According to two recently published articles written by an industry expert, PBMs are stuffing their clients’ contracts with ambiguous contract terms that enable PBMs to rob their clients of a large portion of generic drug savings.

The articles were written by Pharmacy Benefit Consultants' President Linda Cahn, and published serially in the highly regarded magazine, Managed Care. The articles detail the contract ploys that PBMs use to keep a large portion of clients' generic drug savings, and explain exactly what health plans must do to change their contracts, end PBMs’ practices and ensure that health plans retain all available generic drug savings. 

The initial article focuses on PBM’s manipulation of “brand drug” and “generic drug” contract definitions. In the article, Linda Cahn explains that most PBMs purposely write ambiguous contract definitions that enable PBMs to mis-categorize “brand drugs” as “generics”, and “generic drugs” as “brands”, depending on which categorization best suits PBMs’ interests.

For example, when invoicing clients, PBMs categorize numerous “generics” as “brands”, which enables PBMs to provide far weaker discounts to clients when invoicing for millions of generic drug prescriptions. However, when calculating the amount of rebates owed to clients -- or the PBMs’ generic dispensing rates -- PBMs categorize numerous “brands” as “generics”, which enables PBMs to decrease their rebate payments to clients, and inflate their generic dispensing rates.

To end PBMs’ practices, Linda Cahn urges all health plans to demand entirely different definitions in their PBM contracts. "Write airtight definitions for 'brand drug' and 'generic drug in your PBM contracts", says Cahn, "and you'll take the first step toward maximizing your generic drug savings." Cahn's initial article, which can be read here, details exactly how to write different definitions for both contract terms. 

In a second article in Managed Care magazine, Linda Cahn explains additional contract terms that payers must demand to ensure payers obtain all their generic drug savings. Cahn urges payers to require pass-through pricing for all brand and generic drugs -- whether dispensed from retail or mail pharmacies -- meaning that PBMs will have to invoice payers using PBM's actual cost for each retail and mail generic drug dispensed. Cahn also says clients must undergird the contract's pass-through pricing provisions with airtight contract guarantees to ensure the PBM's pass-through pricing will be marketplace competitive.

The article explains that PBM clients should include at least 18 different retail and mail contract guarantees in their PBM contracts, including three generic guarantees in each of the three basic pharmacy channels (retail, retail 90 and mail). The three generic guarantees should be for (i) all generics that are within the 180 day exclusivity period; (ii) all generics that are not within the 180 day exclusivity period; and (iii) an "all-in" generic guarantee for all drugs that are defined as “generic drugs”. Cahn explains that the above guarantees will enable payers to eliminate all “MAC” (maximum allowable cost) terms from their contracts -- which is imperative says Cahn -- since MAC is nothing but a “mousetrap” that PBMs use to charge whatever they want for generic drugs. 

Cahn’s second article, which can be viewed in its entirely by clicking on this link, contains many other generic savings suggestions, including advice to create "maximum per unit costs" for commonly used generics that are the heart of savings programs, and methods to ensure that clients receive the lowest available costs for newly available generic drugs. 

Cahn makes clear why all PBM clients should act now: "In these tough economic times, no health plan can afford to hemorrhage money and squander potential savings from generic drugs," says Cahn. "We urge every health plan to take advantage of the detailed advice provided in these free articles -- by reading the articles, examining your contract to determine which weaknesses it contains, and thereafter demanding entirely different contract provisions from your PBM, either in direct contract negotiations or in PBM RFPs.”

Cahn adds: "RFPs are the single best method to force PBMs to stop stuffing their contracts with ploys that drive up client costs. However, clients must use the RFP's leverage to extract better contract terms from PBMs, rather than simply getting answers to consulting firm questionnaires. Otherwise, RFPs will prove worthless, and clients will end up with yet another contract at the end of their RFPs that is stuffed with loopholes that will drive up their costs."

The Associated Press said Mayor O'Malley made the comments last week and quoted the Democratic gubernatorial candidate as saying that "we must move forward to promote stem cell research that will find cures for diseases like Parkinson's, Alzheimer's and juvenile diabetes."

According to AP, Mayor O'Malley also decried the fact that Baltimore has fallen behind states like California and Massachusetts in enacting legislation that will provide funds for stem cell research, "despite having leading institutions like John Hopkins University, the National Institute of Health and the University of Maryland."

Henry Eawell, a spokesman for Baltimore Governor Robert Ehrlich, told AP that the governor is also committed to supporting stem cell research and "is considering ways right now to empower the Maryland medical community to research potentially life saving medicines," but that there is "considerable debate underway if funding and regulation are best done at the state or federal level." Eawell told AP that the governor has not taken a position on whether he would support state funding for stem cell research at the 2006 legislative season.

Supporters of stem cell research in Baltimore are already trying to build support for bills they will introduce in January to provide aid to research institutions, AP reported.

A new fertility test is being touted as the lastest & greatest. But what many couples need to know before they spend an extra $5,000.00 is that the test is not needed and can be very inaccurate.

For millions of Americans having a family is a life-long dream come true. But for Laguna Niguel resident Sian Shook, and her husband Paul, dreaming of one day raising a family of their own seemed to be just that, a dream. After trying to become pregnant for over a year, Sian 35, finally sought out guidance at a local fertility clinic. There, the doctor insisted that in addition to In Vitro Fertilization the ShookÂ’s spend an extra $5,000.00 to undergo Pre Implantation Genetic Diagnosis (PGD). PGD is a test where one cell is removed from an embryo and tested for its genetic contents prior to transfer back into the uterus.

“The Shook’s were misled into thinking that the PGD test was a necessity,” says Robert Anderson, M. D., founder and medical director of the Southern California Center for Reproductive Medicine. “What they (and millions of women) didn’t know was how inaccurate the PGD test really can be.”

According to Dr. Anderson PGD is used to detect genetic diseases and chromosomal abnormalities in early embryos. When used correctly, it can identify diseases such as cystic fibrosis, hemophilia, Tay-Sachs, muscular dystrophy and many chromosomal abnormalities such as DownÂ’s Syndrom. Confusion arises, however, because doctors can only identify 9 out of the 23 chromosomes that comprise genetic make-up. That leaves14 chromosomes that may have abnormalities, that would be undetected. Recently it has been shown that as many as 50% of abnormalities come from these untested chromosomes. Additionally, research has show that up to 40% of embryos may have more than one genetic cell line, further adding to the confusion.

In Sian ShookÂ’s case, after having the PGD test the doctor told her that only 2 of her 9 eggs were viable and that she could not conceive without the use of an egg donor.

Sian decided to obtain a second opinion and sought out the expert advice of leading fertility specialist, Robert Anderson, M. D. After meeting with Dr. Anderson, he recommended that there was no indication for PGD and advised that to prevent potential damage to any normal embryos from the embryo biopsy procedure, the ShookÂ’s undergo InVitro Fertilization without PGD. After only one attempt of IVF, Sian became pregnant. Sian is now 15 weeks pregnant with her own healthy child and is due to deliver her baby in March 2005.

Dr. Anderson added that routinely using PGD is just another example of some doctorÂ’s using advancements in science in ways that are ethically questionable. Because of the inaccuracies in interpreting the test and the damage that can be done to otherwise normal embryos, he feels that PGD should not be used on a widespread basis. There are limited instances when he recommends patients to have the test. For example, when a woman continually has miscarriages caused by a demonstrated chromosomal abnormality or has a history of a certain genetically transmissible disease in the family, PGD may be of value.

“Although this test is helpful in some situations, the science behind it is still very premature,” commented Anderson. “Until we refine this test, I recommend most patients to spend the extra $5,000.00 on their newborn child!”

PREGNANCY TIMELINE TIPS

1. A quick way to figure out your due date: Add 7 to the number (date in the month) of the first day your period started, then subtract three months and add a year.

2. Due date calculations are typically based on the average 28-day menstrual cycle. However, if your periods are irregular (for example, your cycles vary in length a few days or a week or more) your due date could possibly be off by an equal amount of time.

3. Always remember that a due date is an estimate only, as every baby -- like every human -- is an individual. (Think of it this way: Did you have a growth spurt or start your period at exactly the same time your friends did?)

4. In reality, only about five percent of babies are born on what is technically considered their due dates. Most babies are born anywhere between two weeks before and two weeks after that date -- and they're perfectly term and healthy.

Lo-Cal Ginger Ale

2 liters plain seltzer water
1 tablespoon pressed fresh ginger juice
1 fresh squeezed lemon (medium-sized)
