CDPHP Named First Capital Region Health Plan to Receive Diabetes Incentive Program Endorsement

The New York State Health Foundation (NYSHealth) has named CDPHP one of the first health plans to achieve its new Diabetes Incentive Program Endorsement. As a central component of the Foundation’s New York State Diabetes Campaign, NYSHealth’s Diabetes Policy Center, along with national partner Bridges to Excellence, created the endorsement program to promote financial incentives to providers that drive excellence in diabetes care and outcomes. Only two other health plans serving New York have received this endorsement to date.

The New York State Health Foundation (NYSHealth) has named CDPHP one of the first health plans to achieve its new Diabetes Incentive Program Endorsement. As a central component of the Foundation’s New York State Diabetes Campaign, NYSHealth’s Diabetes Policy Center, along with national partner Bridges to Excellence, created the endorsement program to promote financial incentives to providers that drive excellence in diabetes care and outcomes. Only two other health plans serving New York have received this endorsement to date.

CDPHP has been recognized for demonstrating excellence by offering personal health coaches and classes to members with diabetes, and supplying providers with comprehensive reports on diabetes measures that allow for comparison with colleagues.

The endorsement program is designed to help consumers, providers, and employers identify the health plans that encourage the best diabetes care by offering the most effective incentive and reward programs, as well as helping health plans make improvements to their existing programs.

“Many of the devastating complications diabetes patients suffer from—like heart disease, kidney failure, and loss of limbs—can be prevented with proper treatment. Unfortunately, not enough New Yorkers with diabetes are getting the care they need,” said James R. Knickman, president and CEO of NYSHealth. “Our Diabetes Policy Center established this endorsement program to encourage health plans to reward providers for helping their patients achieve better outcomes. We’re proud to announce that CDPHP has answered that call.”
